Getting started is simple. We provide FDA-approved monitoring devices based on your conditions at no cost to you. Devices include blood pressure monitors for hypertension and heart disease, weight scales for heart failure and kidney disease, glucose meters for diabetes management, pulse oximeters for COPD and lung conditions, and heart rate monitors for cardiac conditions.

Each day, you take quick measurements following your care plan—usually just 1-2 minutes total. Your measurements upload automatically via cellular connection (no WiFi needed, no complicated technology required). Your care team sees your data in real-time and monitors it daily for concerning patterns.

If your readings show troubling trends, your care coordinator reaches out proactively to assess your symptoms, adjust your care plan if needed, coordinate with your doctor, and help prevent a potential emergency before it happens.

After your deductible, Medicare pays 80% and you pay 20% coinsurance. With Medigap or Medicare Advantage, you often pay $0. Most patients pay little to nothing.

Simple Technology, No Technical Skills Required

You don't need WiFi or a smartphone - the devices use built-in cellular connections. You don't need internet or special equipment. Just take your measurements with the device we provide, and everything else happens automatically. If you forget to take a reading, your care coordinator will reach out with a gentle reminder. All data transmission is HIPAA-compliant and encrypted for your privacy.

Combine RPM with Care Management

RPM works even better when combined with Chronic Care Management or Principal Care Management. RPM alone provides vital sign monitoring with clinical oversight. RPM plus CCM or PCM adds vital sign monitoring plus comprehensive care coordination, medication management, appointment coordination, and family support for complete care.
